Internal audit of OHSAS 18001 and ISO 14001

I've been an Internal Auditor for two years. Today, I audited the HR Department. From what I thought going to be a breeze schedule has turned into tight schedule because several documents become non-conformance. 

These are things that keep crossing my mind after the audit:
4.3.1 - hazard identification and risk assessment, 4.4.1 - role and responsibility, 4.4.2 - competence, 4,5,4 - control of records, 4.4.6 - operational control, 4.2 - policy, 4.6 - management review.

Funny, because I can't get these things out of my mind, so I figured out that writing about it would ease my mind.

Texting while driving not just a teen issue

The report from Pew’s Internet and American Life Project found that 27% of adults text while driving –- essentially the same amount of teens who said they do so in another Pew study conducted late last year. But when you measure both sending and reading texts, adults are worse offenders by a margin of 47% to 34%

5 recent major accident (including fatal accident) in Indonesia's mining industry is caused by combination of texting and making a phone call while driving. Due to this matter, we are conducting a campaign of "No Dialing and No Texting while Driving" through out mining operation to reduce the potential risk in the hope to increase the safety awareness, not just for the driver but to passenger as well.

Autonomous Cars will make us safer

Safety regulators are beginning to realize that safety technology designed to minimize injuries, like the ever increasing number of airbags, isn’t paying the dividends it once did. When several tons of vehicle smash together there is only so much energy than can be absorbed, but collision avoidance technologies like electronic stability control are proving to save more lives than any technology since the seatbelt. That’s why automakers like Mercedes-Benz are developing “crash-proof” cars and big players like GM, Volvo and Volkswagen say autonomous cars are just over the horizon.

Substandard action, that is people/person, is the major cause of accident. 98% of accident cause is actually substandard action. In this matter, behavior based safety is much more important to prevent accident.

New York bans texting while driving

New York has joined the ranks of states that ban texting while driving. The interesting part is that you won’t be pulled over for it but you will be fined up to $150 if you are caught texting when they pull you over for another violation

The main problem, I think, is ignorance to safety driving. Fine is a temporary solution, because deep down inside we're the creature who tends to break the law. It's more to culture approach to find a long term solution.
